Welcome to MMA Wellness (“we,” “our,” or “us”). These Terms of Service (“Terms”) govern your access to and use of our website mma-wellness.com (the “Site”) and any services we provide, including appointment scheduling, patient communications, and in-person care. By using our Site or booking an appointment, you agree to these Terms. If you do not agree, please do not use our Site or services.
1. Services Provided
MMA Wellness is a medical clinic offering wellness services including, but not limited to, massage therapy, acupuncture, chiropractic care, and trigger-point injections. Services are provided in accordance with all applicable laws and professional standards.
2. Eligibility
You must be at least 18 years old to book appointments through our Site. Patients under 18 must have a parent or legal guardian’s consent.
3. No Medical Advice Online
Information on this Site is provided for general educational purposes only and is not a substitute for professional medical advice, diagnosis, or treatment. Always seek advice from a licensed healthcare provider regarding any medical condition or treatment.
4. Appointments & Cancellations
By booking an appointment, you agree to our full Cancellation Policy.
Key Points:
- If you are unable to make your appointment due to an emergency, please call us as soon as possible to reschedule.
- If you need to change the time of your appointment, we will try to accommodate you for another time on the same day. If that’s not possible, please try to make up the missed appointment within one week to avoid disrupting your treatment plan.
- 24-Hour Notice Required: We require at least 24 hours’ notice for any cancellations or rescheduling. If less notice is given without a valid emergency, a $30 fee will be added to your account and must be paid before or at your next appointment.
5. Payment
You agree to pay all applicable fees at the time of service unless otherwise arranged. Insurance coverage, when applicable, will be billed according to your plan, but you remain responsible for any amounts not covered.
6. Privacy & HIPAA Compliance
We protect your personal and medical information in accordance with our Privacy Policy and HIPAA regulations. By using our services, you consent to the collection, use, and disclosure of your information as described in our Privacy Policy.
7. Email & Text Communications
By providing your contact information, you consent to receive appointment confirmations, reminders, and important updates via email and/or text message. You may opt out of non-essential communications at any time.
Agreement to Receive Text Messages
By providing your mobile number, you agree that ReGlow Wellness Management DBA MMA Wellness may send you periodic SMS or MMS messages containing but not limited to important information, updates, deals, and specials.
- You will receive up to 4 messages per month.
- You may unsubscribe at any time by texting the word STOP to the (516) 566-0000. You may receive a subsequent message confirming your opt-out request.

- T-Mobile is not liable for delayed or undelivered messages.
- You agree to notify us of any changes to your mobile number and update your account with us to reflect this change.
- Data obtained from you in connection with this SMS service may include your cell phone number, your carrier’s name, and the date, time and content of your messages, as well as other information that you provide. We may use this information to contact you and to provide the services you request from us.
- By subscribing or otherwise using the service, you acknowledge and agree that we will have the right to change and/or terminate the service at any time, with or without cause and/or advance notice.

Will I be charged for the text messages I receive?
Though ReGlow will never charge you for the text messages you receive, depending on your phone plan, you may see some charges from your mobile provider. Please reach out to your wireless provider if you have questions about your text or data plan.

All content on this Site — including text, images, logos, and designs — is the property of MMA Wellness or our licensors. You may not use, reproduce, or distribute our content without prior written permission.
9. Limitation of Liability
To the fullest extent permitted by law, MMA Wellness is not liable for any indirect, incidental, or consequential damages arising from your use of our Site or services.

We may update these Terms from time to time. The updated version will be posted on this page with a revised effective date.
